Printable pet medical record
A free two-page medical record for your dog or cat: a PDF you can fill in on screen or print and complete by hand. Allergies, medicines, vet visits, weight and test results sit in one place, so a new vet, a sitter or you at 2 a.m. can find them in a minute.

What’s inside
- Page 1: your pet’s name, species and breed, sex, date of birth, color and microchip number, your contact details and pet insurance.
- Two blank boxes for allergies and long-term conditions. They are empty on purpose: write down what your vet told you, in your own words.
- A table of current medications: medicine, dose, how often, since when and who prescribed it.
- Phone numbers of your regular vet, the nearest emergency clinic and anyone else who looks after your pet.
- Page 2: a vet visit log with the date, the clinic, the reason and what was prescribed or advised.
- A weight table next to a list of tests and documents, with where each one is kept. Weight is in pounds on Letter and kilograms on A4.
How to use it
- Fill it in on screen or by hand. The fields work in Adobe Acrobat Reader, Preview on a Mac and most browser PDF viewers; save the file after typing.
- Start with page 1. Copy the microchip number and any allergies from your vet’s records or invoices, not from memory.
- After each visit, add a line to the visit log while the details are fresh, and update the medications table if anything changed.
- Weigh your pet on the same scale at about the same time of day, and write the date next to each weight.
- When you get a test result or a certificate, list it under “Tests and documents” and note where the original is: a folder, an email, the clinic.
- Take the record to vet visits, boarding and a new clinic, and leave a copy with your pet sitter.
When it helps most
A new vet or an emergency clinic at night, a move to another city, a long-term condition with several medicines, or a pet who stays with family while you travel. Each time, the person caring for your pet gets the history in a minute instead of calling you.
Medical record or vaccination record?
Vaccinations have their own form: our free vaccination record, with lot numbers and a box for the vet’s signature or stamp. The medical record covers everything else. Print both and keep them in one folder.
Important
This is your own record. It does not replace medical records, prescriptions or certificates issued by a veterinarian, and it is not proof of vaccination. If anything here differs from what your vet wrote, the vet’s records are the ones that count.
