Printable cat health record
A free two-page health record for your cat, to fill in on screen or by hand. Allergies, medicines, vet visits, weight and test results stay in one place, so a cat sitter, a cattery or a new vet has the history without calling you.

What goes on each page
- Page 1: your cat’s name, breed, sex and neutering, date of birth, coat and microchip number, your details and pet insurance.
- Two empty boxes for allergies and long-term conditions, filled in with what your vet has told you.
- Current medicines, including a prescription diet if your vet set one, with the dose, how often and since when.
- Your vet, the nearest emergency clinic and your cat sitter, each with a phone number.
- Page 2: every vet visit with the reason and what was prescribed, then weight and test results side by side.
Small changes, written down
Changes in a cat are often quiet: a little less eaten, a new habit at the litter box, a few ounces gone over a season. When you call the vet, dates and numbers say more than “I think she’s eating less”. Add a line to the visit log with the day you first noticed it, and bring the record along.
Weighing a cat at home
- Weigh yourself on a bathroom scale, then step on again holding your cat, and subtract.
- Or weigh your cat in its carrier and take off the weight of the empty carrier.
- Use the same scale every time and write the date next to each number.
- Pounds go on the Letter version, kilograms on A4.
- If a number surprises you, ask for a weigh-in on the clinic scale at the next visit.
When it helps most
A sitter who drops in twice a day, a week at a cattery, a move to a new city and a new vet, an emergency clinic at night, or a course of pills that has to be given on time. Each time, whoever is looking after your cat sees the allergies, the medicines and your vet’s number at a glance.
Health record or vaccination record?
Vaccinations have their own form, with lot numbers and a box for the vet’s signature: the cat vaccination record. Keep both in one folder next to the carrier.
Important
This is your own record of what your vet told you. It does not replace the clinic’s medical records, prescriptions or certificates, and it is not proof of vaccination. Where it differs from what the vet wrote, the vet’s records count.
